Health Professional Educators in Family Medicine (HPE-FM) Department Representative Annual Report

Responsibilities and Expectations of Health Professional Educators in Family Medicine (HPE-FM):

1. Identify and document the HPE-FMs who are contributing to FM teaching in a meaningful way in each department.

2.  Link those HPE-FMs to the HPE-FM HUB, resource and communication portal with the College of Family Physicians of Canada (CFPC)

3. Complete the HPEG-FM Department representative annual report form below, submit it to your department chair and 'cc' it to the HPEG-FM Exec

4. Attend (either in person or virtually) the annual HPEG-FM meeting which occurs on the Friday of each annual FMF conference

5. Whenever relevant, liaise with the appropriate educators within departments in order to facilitate connecting with as many of the HPE-FMs as possible

6. Please identify your own relationship to your Department of Family Medicine (i.e. full-time faculty, part-time faculty, adjunct appointment, clinical appointment, etc.)
